MDS Coordinator RN

Our client, a well-established Skilled Nursing and Rehabilitation facility in Dekalb, Illinois, is seeking an experienced MDS Coordinator RN to join their clinical leadership team. This is an excellent opportunity for a detail-oriented Registered Nurse with strong knowledge of the MDS/RAI process, Medicare PPS, and Medicaid reimbursement to make a meaningful impact on quality outcomes and compliance.
Job Summary
The MDS Coordinator RN is responsible for the accurate and timely completion of the Minimum Data Set (MDS) and management of the RAI process from admission through discharge. This role partners closely with nursing leadership and the interdisciplinary team to ensure regulatory compliance, optimized reimbursement, and resident-centered care.
Key Responsibilities

Complete and manage all MDS assessments in compliance with CMS, OBRA, and state regulations

Oversee the RAI process from admission to discharge

Ensure accuracy with Medicare PPS, Case-Mix, and Medicaid reimbursement

Monitor Quality Measures and Quality Indicators

Participate in and support the care planning process

Track admissions, discharges, and MDS scheduling

Collaborate with nursing leadership to support clinical quality and compliance

Educate clinical team members on MDS processes as needed
Qualifications

Active Registered Nurse (RN) license in the state of Illinois

Prior MDS Coordinator or MDS experience strongly preferred

Minimum of one year of experience in a LTC / SNF / Skilled Nursing setting preferred

Strong working knowledge of MDS 3.0, OBRA regulations, and Quality Measures

Experience with Medicare and Medicaid reimbursement

Excellent attention to detail and organizational skills

Strong written and verbal communication skills
