Job Responsibilities

As the MDS Coordinator (LPN or RN), you will:

  • Complete and manage MDS assessments in compliance with federal, state, and CMS/OBRA regulations

  • Oversee the RAI process from admission through discharge

  • Track admissions, discharges, and MDS scheduling

  • Ensure accuracy related to Medicare PPS, Case-Mix, and Medicaid reimbursement

  • Participate in interdisciplinary care planning and quality initiatives

  • Support clinical documentation systems and staff education as assigned

  • Maintain accurate documentation to support regulatory compliance

Qualifications

  • Active LPN or RN license in Missouri (required)

  • Prior MDS coordination experience strongly preferred

  • Minimum 1 year of experience in LTC, SNF, Assisted Living, or Memory Care preferred

  • Knowledge of MDS 3.0, Quality Measures, OBRA, Medicare PPS, Medicaid reimbursement, and Case-Mix

  • Strong organizational and communication skills

  • Detail-oriented, dependable, and resident-focused
