A well-established skilled nursing and rehabilitation community in Forsyth, Missouri is hiring an experienced MDS Coordinator to support clinical compliance, care planning, and reimbursement accuracy. Job Responsibilities

As the MDS Coordinator, you will:

  • Coordinate and complete the RAI/MDS process from admission through discharge

  • Ensure compliance with federal, state, and regulatory requirements

  • Manage MDS scheduling, submissions, and tracking of assessments

  • Monitor admissions, discharges, and assessment timelines

  • Collaborate with the interdisciplinary care team on care planning and quality initiatives

  • Support nursing leadership with compliance, quality measures, and reimbursement accuracy

  • Assist with clinical documentation systems and staff education as needed


Qualifications

  • Active LPN or RN license in Missouri (required)

  • MDS experience strongly preferred

  • Minimum 1 year experience in SNF, LTC, Assisted Living, or Memory Care preferred

  • Knowledge of Medicare PPS, Medicaid reimbursement, Case-Mix, OBRA, and Quality Measures

  • Understanding of interdisciplinary care planning

  • Strong organizational, written, and verbal communication skills

  • Professional, dependable, and resident-focused
