MDS Coordinator | RN – Cedar Rapids Area

Full-Time | Skilled Nursing & Rehabilitation Facility

Our client, a skilled nursing and rehabilitation community in the Cedar Rapids area, is seeking an experienced MDS Coordinator | RN to join their clinical team. What You’ll Do

As the MDS Coordinator | RN, you will:

  • Complete and manage all MDS assessments in compliance with federal, state, and certification guidelines

  • Oversee the RAI process from admission through discharge

  • Track admissions, discharges, and MDS schedules

  • Ensure compliance with Medicare PPS, Case-Mix, and Medicaid reimbursement

  • Support interdisciplinary care planning and quality initiatives

  • Assist with clinical information systems and staff education as directed

  • Collaborate with nursing leadership to maintain quality outcomes and regulatory compliance


Qualifications

  • Licensed as a Registered Nurse (RN) in the state you practice

  • Prior MDS coordination experience highly preferred

  • Minimum of one year experience in LTC, SNF, AL, or MC settings preferred

  • Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations, and Quality Indicators

  • Understanding of the care planning process

  • Strong written and verbal communication skills

  • Professional, detail-oriented, and passionate about working with seniors

  • Energetic, flexible, and personable
