Our client is seeking a Wound Nurse | Licensed Practical Nurse (LPN or RN) to join a skilled nursing and long-term care community in South Beloit, Illinois. This role is ideal for an LPN with wound care experience who is passionate about resident-centered care and clinical excellence.

Key Responsibilities

  • Manage and oversee the wound care and skin management program

  • Assess, treat, and monitor wounds including pressure injuries, surgical wounds, ulcers, feeding tube sites, and abscesses

  • Develop and implement individualized wound care treatment plans

  • Perform wound documentation and ensure compliance with care standards

  • Monitor for infection, healing progression, and treatment effectiveness

  • Collaborate with nursing leadership and interdisciplinary care teams

Qualifications

  • Active Licensed Practical Nurse (LPN) license (Illinois)

  • Wound care experience required

  • Minimum 1 year of clinical experience in Skilled Nursing, Long-Term Care, Assisted Living, or Memory Care

  • Strong knowledge of wound assessment, treatment, and documentation

  • Excellent communication and organizational skills
