Wound Nurse (RN) – Long Term Care

Our client, a well-established long-term care and senior living community Just West of Chicago, Illinois, is hiring an experienced Wound Care Nurse (RN). This position is ideal for a licensed practical nurse with hands-on wound care experience in LTC, SNF, Assisted Living, or Memory Care who is passionate about skin integrity and quality outcomes.


Job Overview

The Wound Care Nurse (RN) is responsible for coordinating, assessing, and delivering wound care treatments while supporting compliance, documentation, and resident-centered care. This role works closely with nursing leadership and interdisciplinary teams to reduce pressure injuries and promote healing.


Essential Duties & Responsibilities

  • Provide wound care treatments including pressure ulcers, surgical wounds, diabetic ulcers, and feeding tube sites

  • Assess, stage, document, and monitor wounds and skin integrity

  • Develop, implement, and evaluate wound care treatment plans

  • Monitor for infection, changes in condition, and healing progress

  • Ensure accurate and timely clinical documentation

  • Collaborate with DON, nursing staff, physicians, and care teams

  • Support regulatory compliance and best practices in wound management


Required Qualifications

  • Active Illinois Registered Nursing (RN) license

  • Wound care experience required

  • Minimum 1 year of experience in Long-Term Care (LTC), Skilled Nursing Facility (SNF), Assisted Living, or Memory Care

  • Strong clinical assessment and documentation skills

  • Knowledge of pressure injury prevention and treatment

  • Excellent communication and teamwork abilities


Preferred Experience (SEO Boost)

  • Certified Wound Care Nurse (preferred, not required)

  • Experience with skin rounds and wound audits

  • Familiarity with state survey standards and infection prevention
