LeaderStat is partnering with Continental Senior Communities to identify a skilled Wellness Director for Ashton Grove Senior Living. This role is ideal for a nurse leader who thrives in a hands-on environment and values both clinical excellence and resident connection.
As Wellness Director, you’ll be the driving force behind the health and well-being of residents—leading the care team, setting clinical standards, and ensuring every resident receives personalized, high-quality support.

Your Day-to-Day
- Direct and oversee all resident care services and clinical processes
- Guide and support nurses and caregivers in delivering exceptional care
- Conduct assessments and ensure individualized care plans are followed
- Monitor resident wellness and respond proactively to changes in condition
- Maintain compliance with state regulations and company standards
- Serve as a trusted resource for residents, families, and staff
- Partner closely with the Executive Director on overall community success
What We’re Looking For
- Licensed RN or LPN (Kentucky)
- Experience in Assisted Living or Memory Care strongly preferred
- Prior experience supervising or leading a care team
- Strong clinical judgment and attention to detail
- Ability to balance compassion with accountability
- A steady, dependable leader who leads by example
