RN MDS Coordinator – Continuing Care Retirement Community

Nestled in the heart of America’s Historic Triangle and just minutes from downtown Colonial Williamsburg, our premier not-for-profit continuing care retirement community offers an exceptional environment for residents and team members alike. We are seeking a dedicated and detail-oriented RN MDS Coordinator to join our well-established clinical team.

Position Overview

The RN MDS Coordinator is responsible for monitoring the quality of life and clinical outcomes for residents in Health Services. Working collaboratively with an interdisciplinary team, the RN MDS Coordinator develops individualized plans of care to ensure the highest standards of service. This position oversees timely and accurate MDS assessments, ensures care plans are completed, supports accurate clinical documentation, and assists with infection control initiatives in partnership with the Director of Nursing.

This is a non-floor position — no direct floor assignments. You’ll work alongside a tenured, well-staffed clinical team with a strong survey history and healthy occupancy levels.


Key Responsibilities

  • Complete, coordinate, and submit MDS assessments in accordance with all federal and state guidelines.

  • Collaborate with all clinical disciplines to develop and maintain individualized, resident-centered care plans.

  • Ensure documentation accuracy and compliance across the care team.

  • Monitor residents’ quality of life and support clinical decision-making.

  • Provide support in infection control activities in collaboration with the DON.

  • Maintain exceptional standards of regulatory compliance and best practices.

    Qualifications

    • Active RN license in Virginia or multi-state compact (in good standing)

    • Current CPR/AED certification

    • Minimum 1 year of MDS experience in a SNF or long-term care setting

    • RAC-CT certification (or willingness to obtain)

    • Experience with EHR systems; PointClickCare preferred

    • Wound Care Certification (or willingness to obtain)

    • Strong clinical assessment skills and working knowledge of federal & state regulations

    • Excellent communication and interpersonal skills; able to collaborate effectively with an interdisciplinary team
