Position Overview

The RN MDS Coordinator plays a critical role in ensuring accurate, timely, and compliant completion of MDS assessments and care plans. This position works closely with interdisciplinary team members to support resident-centered care while optimizing clinical documentation and reimbursement outcomes.

Key Responsibilities

  • Coordinate and manage the full RAI/MDS assessment process

  • Ensure MDS assessments and care plans are completed accurately and within required timelines

  • Maintain compliance with federal and state regulations, Medicare, and Medicaid guidelines

  • Collaborate with interdisciplinary team (IDT) members to collect assessment data and support care planning

  • Monitor clinical documentation for accuracy, consistency, and alignment with MDS data

  • Submit MDS data to appropriate agencies in accordance with regulatory deadlines

  • Educate clinical staff on MDS processes, documentation standards, and best practices

  • Participate in audits, quality improvement initiatives, and survey preparedness efforts

Qualifications

  • Active Ohio RN license in good standing

  • 2+ years of MDS Coordinator experience in a skilled nursing setting

  • Strong knowledge of Medicare PPS, Medicaid, and reimbursement systems

  • Highly organized with strong analytical and communication skills

  • Experience with EMR systems such as PointClickCare, MatrixCare, or similar platforms

  • RAC-CT certification preferred
