Clinical Supervisor - Home Health (RN)

Position Summary
The Clinical Supervisor leads and supports the home health clinical team, ensuring high-quality patient care, regulatory compliance, clinical excellence, and efficient operations. This role oversees care planning, documentation, staff development, quality assurance programs, and clinical performance while promoting positive patient outcomes and organizational success.
Key Responsibilities
Clinical Leadership & Operations
- Supervise and guide home health clinical staff
- Conduct supervisory patient visits and assess staff competency
- Oversee patient care services and ensure care plans are implemented effectively
- Review and approve clinical documentation and workflows
- Monitor operational performance, staffing needs, productivity, and census trends
- Collaborate with referral sources and internal teams to ensure timely patient admissions and services
Quality & Compliance
- Audit medical records and clinical documentation for accuracy and regulatory compliance
- Ensure adherence to Medicare, state, federal, and accreditation standards
- Provide ongoing education and coaching related to clinical documentation and regulatory requirements
- Lead survey readiness efforts and compliance audits
- Develop corrective action plans and quality improvement initiatives
Team Development
- Recruit, onboard, train, and mentor clinical team members
- Foster a positive and engaging work environment
- Conduct performance evaluations and provide coaching for professional growth
- Support staffing and scheduling needs to ensure continuity of care
- Ensure accurate and timely payroll processing and performance management activities
Qualifications
- Current, unrestricted Ohio Registered Nurse (RN) license
- Minimum of 3 years of clinical operations supervisory experience
- Minimum of 1 year of home health experience
- Minimum of 1 year of experience in a Medicare-certified home health agency
- Proficiency with Microsoft Office applications and internet-based systems
- Valid driver's license and acceptable driving record
- Occasional on-call responsibilities
- May work weekends, evenings, and holidays as needed for coverage
