JOB DESCRIPTION
POSITION: Qualified Intellectual Disability Professional (QIDP)

POSITION SCOPE:
The Mary Campbell Center is home to 68 residents. The QIDP is responsible for coordinating active treatment and related services for residents and ensuring they are delivered with a high degree of quality in a homelike environment where MCC residents live with respect, personal dignity, and independence to the fullness of their abilities. The QIDP acts as the resident’s primary advocate in coordinating services as the QIDP, ensuring an individualized and comprehensive active treatment program appropriate to each resident in an assigned caseload. The QIDP is charged with championing the Guiding Principles and Mission of The Center in a manner that ensures compliance with State and Federal regulations. The QIDP must be a positive role model who reliably works effectively, efficiently, and with respect for residents, staff, families, volunteers, professional consultants and the physical environment of The Center.
MINIMUM REQUIREMENTS:
The QIDP at the Mary Campbell Center must 1) be a good person of high character who demonstrates compassion and understanding, consistently works with a positive attitude and advocates effectively on behalf of residents who have varying degrees of disabilities; 2) possess a Bachelor in Social Work, Human Services, Special Education or related field from an accredited college or university; 3) possess experience in a social services setting working with people with disabilities; 4) must have at least one year of employment working directly with individuals with intellectual disabilities or developmental disabilities; 5) exhibit strong interpersonal skills, and effective written and verbal communication skills; 6) exhibit proficiency in utilizing technology tools to manage program reporting and achieve individual, department and mission goals, and 7) be at least 21 years of age with a valid driver’s license and clean driving record.
ESSENTIAL FUNCTIONS:
1. Coordinates and facilitates Individual Program Plan (IPP) annual review meetings according to established policy and procedures.
2. Monitors resident progress of Individual Program Plan and intervenes in cases where progress is not evident, essential programs are lacking or quality is not consistent with IPP goals, including but not limited to bi-monthly documentation reviews and program observations.
3. Establishes a positive working relationship with each assigned resident, demonstrating compassion, engagement, and eagerness toward assisting them in meeting their active treatment goals.
4. Develop and implement resident Social Support Plans for the residents who are identified as having psycho-social needs.
5. Completes all documentation in a timely fashion according to established policy and procedures including progress notes on all assigned residents. Effectively communicates critical information about residents’ status.
6. Maintains open communication with families, providing support and encouragement toward ongoing interaction with resident.
7. Serves as a facilitator and resource for staff to address all residents’ active treatment needs.
8. Ensure that adequate environmental supports and assistive devices are present to promote resident independence.
9. Assists with the investigation, documentation and resolution of incidents involving reported cases of abuse, neglect and exploitation of resident property.
10. Completes a discharge plan/summary and follow-up as needed.
11. Facilitates and/or participates in small groups to meet resident presenting needs.
12. Systematically reviews and assists residents when necessary with personal funds, bills, spending, and escrow account statements, etc.
13. Provides opportunity to review advanced health care directives.
14. Conducts quarterly site visits for residents working or volunteering in community programs.
15. Attends and actively participates in the following committees: Mental Health Committee, Human Rights, Interdisciplinary Team, and other committees as assigned. (Wheelchair Safety, Safety, Active Treatment, HIPAA, MIS, Environmental Rounds, etc.)
16. Coordination & leadership of Seasonal Clothing Changeovers and room-moves as needed. Monitors resident adjustment process to room moves.
17. Lead monthly Resident Neighborhood Meetings and documents attendance & minutes as assigned.
18. May be required to transport residents in the community utilizing Center vehicles.
19. Attends continuing education experiences as approved by supervisor.
20. Demonstrates proficiency utilizing computer systems and networks.
21. Completes all mandatory in-services and attends staff meetings as required.
22. Performs other duties as assigned.
