Qualifications:
- TRULY Board Eligible (within 5 years of eligibility with plans to test) or Board Certified in Vascular Surgery (*no exceptions)
- OH license (or willing to obtain)
- DEA (must be obtained prior to start) (*no exceptions)
- Clean malpractice and license (*no exceptions)
- Most recent NPDB Self Query Report (within 30 days active) (*no exemption)
