Qualifications:
- Board Eligible OR Board Certified in Otolaryngology (ENT)
- Active CO License (or willingness to obtain)
- Clean malpractice/license history
- Most recent NPDB Self Query Report (within 30 days active) *no exemption
- DEA (must be obtained prior to start) *no exemptions
Salary Range: $458,000 - $508,000
