MDS Coordinator – RN or LPN

LeaderStat is partnering with a well-established nonprofit skilled nursing and long-term care community in Baltimore, MD, to hire an experienced MDS Coordinator. This position is open to either an RN or LPN with strong MDS experience who can step into the role and make an immediate impact.
What You'll Do
- Coordinate and complete the MDS/RAI process in accordance with regulatory requirements.
- Complete initial, quarterly, annual, and other required resident assessments.
- Review clinical documentation to ensure accuracy, compliance, and appropriate reimbursement.
- Complete, lock, and transmit MDS assessments as required.
- Participate in care planning, clinical meetings, and triple-check meetings.
- Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
- Identify changes in resident condition and communicate concerns and recommendations to the appropriate clinical leaders.
- Review staff documentation and provide education to ensure compliance with regulatory and reimbursement requirements.
- Educate the care team on MDS processes and documentation.
- Participate in surveys, inspections, and plans of correction as needed.
- Ensure documentation appropriately supports resident care and reimbursement.
What We're Looking For
- Current RN or LPN license in good standing with the Maryland Board of Nursing.
- 3+ years of MDS experience in a skilled nursing/long-term care setting.
- Strong knowledge of the MDS/RAI process and clinical documentation requirements.
- Current AHA Healthcare Provider BLS certification.
- Strong organizational and communication skills.
- Ability to work independently and collaborate effectively with an interdisciplinary team.
- RAC-CT certification is not required at hire, but willingness to obtain it is expected.
This is an onsite position with no on-call requirement. The ideal candidate will have the experience and confidence to hit the ground running rather than needing extensive MDS training.
