Position Summary
The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) assessments, Care Area Assessments (CAAs), care planning, and regulatory documentation. This role ensures accurate and timely assessment completion, optimal reimbursement outcomes, compliance with federal and state regulations, and collaboration with the interdisciplinary care team to support quality resident care.
The ideal candidate is a detail-oriented, organized, and resident-focused nurse with strong analytical, communication, and leadership skills.
Essential Responsibilities
- Complete and submit MDS assessments, including:
- Admission assessments
- Quarterly reviews
- Significant change assessments
- Annual assessments
- Discharge assessments
- Coordinate and audit MDS documentation, CAAs, and individualized care plans with the interdisciplinary team
- Ensure timely and accurate electronic submission of all resident assessments
- Facilitate care plan meetings and communicate resident care needs with families, caregivers, and team members
- Develop, implement, and maintain individualized resident care plans
- Monitor documentation practices to ensure compliance with CMS, state, and facility requirements
- Maintain responsibility for ICD coding and related documentation
- Coordinate physician certifications and regulatory documentation
- Manage insurance authorizations, updates, and communication with payers as needed
- Develop and maintain expertise in RUG-IV and reimbursement methodologies to support accurate resident assessment and reimbursement
- Schedule Assessment Reference Dates (ARDs) appropriately to maximize assessment accuracy and reimbursement opportunities
- Address resident concerns and family questions regarding assessments, care plans, and services
- Maintain proficiency with the facility's clinical and assessment software systems
- Participate in quality improvement initiatives and regulatory survey preparedness activities
Qualifications
- Graduate of an accredited nursing program
- LPN or RN license required
- Previous MDS coordination experience preferred
- Knowledge of restorative nursing techniques and their practical application
- Experience in long-term care, skilled nursing, rehabilitation, or senior living preferred
- Understanding of Medicare, Medicaid, and reimbursement processes preferred
- Proficiency with electronic medical records and MDS software systems
