As an LPN/RN, you will provide high quality nursing care under the direction of nursing and medical staff. You’ll play a key role in administering medications, performing treatments, maintaining accurate documentation, and supporting residents’ overall well being in a respectful and dignified environment.

Responsibilities

  • Deliver nursing care that reflects our mission, vision, and values
  • Administer medications and treatments using electronic medical records
  • Participate in resident assessments, care planning, interventions, and evaluations
  • Observe residents and report changes or adverse reactions promptly
  • Assist physicians and medical professionals during resident visits
  • Collect specimens and perform routine laboratory tests
  • Coordinate ancillary care visits (podiatry, dental, optometry, audiology, etc.)
  • Ensure accurate, timely documentation and reporting
  • Promote resident rights, privacy, and confidentiality
  • Follow infection control, safety, and disaster procedures
  • Collaborate with interdisciplinary teams to ensure quality care
  • Assist residents with mobility, transfers, and daily needs
  • Support QAPI initiatives and continuous improvement efforts
  • Help orient and mentor nurses and nursing assistants
  • Accept reassignment to other units as needed

Leadership & Supervision:

  • Supervise nursing assistants on assigned unit
  • Assist with training, scheduling, performance evaluations, and staff support
  • Address concerns and help maintain a positive, team?focused environment

Qualifications:

  • Graduate of an accredited Nursing program
  • Active LPN/RN license
  • IV Certification preferred
  • Experience in long term care or assisted living preferred
  • Strong communication, organization, and teamwork skills
  • Commitment to compassionate, resident centered care
