Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes on an intermittent basis under the direction and supervision of a Registered Nurse (RN). Care is delivered in accordance with physician orders, individualized care plans, and agency policies and procedures.
Responsibilities
- Provide skilled nursing services according to physician orders and patient care plans.
- Administer medications and treatments as prescribed.
- Monitor patient conditions and promptly report changes to the RN and physician.
- Assist patients and families with education regarding disease management, medications, and self-care techniques.
- Maintain accurate, timely, and complete clinical documentation.
- Collaborate with physicians, RNs, therapists, and other healthcare professionals to coordinate patient care.
- Participate in patient care conferences and interdisciplinary team meetings.
- Support preventive, restorative, and rehabilitative nursing interventions.
- Follow infection control and aseptic techniques.
- Assist with medical record reviews, quality improvement initiatives, and agency compliance activities.
- Respond to on-call assignments, including nights, weekends, and holidays as scheduled.
- Ensure patient confidentiality and compliance with HIPAA regulations.
- Perform other duties as assigned.
Qualifications
Required
- Graduate of an accredited Practical Nursing program.
- Current and unrestricted LPN license in the state of practice.
- Current CPR certification.
- Strong clinical assessment and communication skills.
- Ability to work independently while managing multiple priorities.
- Reliable transportation and valid driver's license.
Preferred
- At least one (1) year of nursing experience.
- Previous home health experience.
