Licensed Social Worker - Skilled Nursing

Our client is a compassionate, patient-centered skilled nursing facility dedicated to providing high-quality care to our residents. Their multidisciplinary team is committed to promoting dignity, respect, and wellness in a supportive environment.

Position Summary:

We are seeking a Licensed Social Worker (LSW) with experience in skilled nursing or long-term care to join our team. The ideal candidate will have a strong understanding of psychosocial needs in geriatric populations, discharge planning, and regulatory compliance.

Key Responsibilities:

  • Conduct initial and ongoing psychosocial assessments of residents.

  • Develop individualized care plans and participate in interdisciplinary team meetings.

  • Assist with discharge planning and coordination of post-discharge services.

  • Provide emotional support and counseling to residents and families.

  • Advocate for residents' rights and help resolve grievances.

  • Maintain accurate and timely documentation in accordance with state and federal regulations.

  • Collaborate with medical staff, nursing, therapy, and community agencies.

  • Participate in care conferences and quality assurance initiatives.

Qualifications:

  • Required: Active LSW (Licensed Social Worker) license in the state of Ohio.

  • Bachelor’s or Master’s Degree in Social Work from an accredited institution.

  • At least 1 year of experience in a skilled nursing facility or healthcare setting preferred.

  • Knowledge of Medicare/Medicaid regulations and discharge planning.

  • Strong communication, organization, and interpersonal skills.

  • Ability to work independently and collaboratively in a fast-paced environment.
