We are seeking an experienced Lead RN MDS Coordinator to join our well-established, not-for-profit Continuing Care Retirement Community (CCRC) in Fort Wayne, Indiana. What You’ll Do:
- Lead and oversee the MDS process for a large, skilled nursing census
- Ensure timely and accurate completion of MDS assessments in compliance with state and federal regulations
- Drive care planning and collaborate with interdisciplinary teams
- Monitor case mix index (CMI) and support reimbursement optimization
- Serve as a resource and mentor for clinical team members
- Maintain survey readiness and regulatory compliance
What We’re Looking For:
- Active RN license in the state of Indiana
- Proven experience as an MDS Coordinator in a skilled nursing or CCRC setting
- Ability to manage a large caseload independently with minimal oversight
- Strong understanding of Medicare/Medicaid, PDPM, and reimbursement processes
- Detail-oriented with excellent organizational and leadership skills
- Comfortable working in a faith-forward environment (participation in faith activities is not required, but comfort with daily Mass and prayer is important)
