We are seeking an experienced Lead RN MDS Coordinator to join our well-established, not-for-profit Continuing Care Retirement Community (CCRC) in Fort Wayne, Indiana. What You’ll Do:

  • Lead and oversee the MDS process for a large, skilled nursing census
  • Ensure timely and accurate completion of MDS assessments in compliance with state and federal regulations
  • Drive care planning and collaborate with interdisciplinary teams
  • Monitor case mix index (CMI) and support reimbursement optimization
  • Serve as a resource and mentor for clinical team members
  • Maintain survey readiness and regulatory compliance

What We’re Looking For:

  • Active RN license in the state of Indiana
  • Proven experience as an MDS Coordinator in a skilled nursing or CCRC setting
  • Ability to manage a large caseload independently with minimal oversight
  • Strong understanding of Medicare/Medicaid, PDPM, and reimbursement processes
  • Detail-oriented with excellent organizational and leadership skills
  • Comfortable working in a faith-forward environment (participation in faith activities is not required, but comfort with daily Mass and prayer is important)
