Occupational Therapist (OT) – Home Health
The Occupational Therapist provides skilled occupational therapy services to patients in their homes, in accordance with physician orders and the established plan of care, collaborating with physicians, caregivers, and the interdisciplinary team to improve patient safety, independence, and functional outcomes.Qualifications – Required
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in the State of Colorado
- Current CPR certification, or ability to obtain upon hire
- Valid driver's license, reliable transportation, and automobile insurance
- Home Health experience
- OASIS experience
- Two (2) or more years of Occupational Therapy experience
- Perform initial evaluations and ongoing reassessments; develop individualized treatment plans
- Provide skilled occupational therapy services in the patient's home
- Educate patients, families, and caregivers on home exercise programs, safety, adaptive equipment, and ADLs
- Collaborate with physicians, nurses, case managers, and the care team; complete OASIS assessments and ICD-10 coding
- Supervise COTAs and Home Health Aides as assigned
- Prepare timely documentation, participate in QAPI and care conferences, and maintain regulatory compliance

Physical Therapist (PT) – Home Health
The Physical Therapist administers physical therapy to patients on an intermittent basis in their place of residence, in accordance with physician orders and the plan of care, under the direction of the Clinical Manager/Nursing Supervisor. This is an in-person role only.Qualifications
- Graduate of a physical therapy curriculum approved by the APTA or an equivalent accrediting body
- Currently licensed to practice in Colorado
- Two (2) years of experience preferred
- Ability to exercise initiative and independent judgment
- Evaluate patient level of function; develop and revise the plan of care with the physician
- Perform skilled physical therapy procedures as ordered; consult with physicians on treatment changes
- Instruct patients and families in home programs and activities of daily living
- Supervise PTA (every 30 days) and Home Health Aide personnel as assigned
- Complete OASIS assessments, recertifications every 60 days, and discharge summaries
- Prepare timely clinical and progress notes; utilize ICD-10 diagnoses and coding
- Participate in peer review, utilization review, QAPI activities, and staff meetings

Speech Language Pathologist (SLP) – Home Health
The Speech Language Pathologist provides skilled speech therapy services to patients in their homes, evaluating and treating communication, cognitive, language, voice, and swallowing disorders while collaborating with the interdisciplinary care team to maximize patient outcomes.Qualifications – Required
- Master's Degree in Speech Language Pathology from an accredited program
- Current Speech Language Pathologist license in the State of Colorado
- Certificate of Clinical Competence (CCC-SLP) preferred
- Current CPR certification, or ability to obtain upon hire
- Valid driver's license, reliable transportation, and automobile insurance
- Home Health experience
- OASIS experience
- Two (2) or more years of Speech Therapy experience, including adult and geriatric populations
- Complete evaluations, reassessments, and individualized treatment plans
- Assess and treat speech, language, communication, cognitive, voice, and swallowing (dysphagia) disorders
- Educate patients, families, and caregivers on communication strategies and swallowing precautions
- Collaborate with the care team; complete OASIS assessments and ICD-10 coding
- Prepare timely clinical documentation and participate in QAPI, utilization review, and care conferences

Work Environment (All Positions)
- Home Health field position with travel between patient homes within the assigned service area
- Occasional attendance at agency meetings, training, and care conferences
