LeaderStat is partnering with a non-profit, award-winning, senior living provider who has been providing top-notch quality care for 50 years! We are seeking a Health and Wellness Navigator for the independent living residents at their retirement community in Westchester County. It is a University Retirement Community, one of the first few in the nation.
The Health and Wellness Navigator will be responsible for evaluating residents’ needs, identifying, and assisting them in accessing the available resources within and outside the community to reduce barriers to successful independent living. This person will ensure a seamless transition between appropriate levels of care offered at the community by building relationships, solving problems, and locating resources for residents transitioning throughout the continuum of care.

Health and Wellness Navigator qualifications:
- Degree in Social Work or RN in New York (active and in good standing)
- At least 2 years of experience working in a long-term care, senior living or hospital setting
- At least 3 years of organizational or case management experience with an interdisciplinary team approach
- Strong interpersonal skills including the ability to motivate and encourage residents to achieve maximum independence and quality of life
- Ability to problem-solve, make ethical recommendations, define appropriate boundaries and be resolution oriented.
- Excellent verbal and written communication skills for groups and individuals of different populations including persons with disabilities and Dementia.
- Superior organizational skills (delegation, time management, multi-tasking)
- Ability to work collaboratively with community management, internal and external resources/partners and communicate with residents, families, physicians and staff.
