The organization is located in the greater Santa Fe, NM area and so requires candidates who live within a reasonable commute of the on-site work location.
This position focuses on assessments, care planning, medication oversight, incident management, regulatory compliance, family communication, and clinical monitoring to ensure safe, high-quality, person-centered care. The Clinical Care Coordinator partners closely with other staff and healthcare providers, families, and operational leadership to identify clinical needs, implement appropriate interventions, and maintain compliance with regulatory and organization standards.
Knowledge of and experience with State and Federal regulations for Home Care and Memory Care are an essential skill for any potential candidate.
You'll be developing care plans and implementing them following your assessments, in partnership with the care team. You will establish protocols to ensure safe medication management, prompt documentation, and regulatory compliance. You'll ensure your team is well versed in all stages of care and how to best meet the needs of residents/ patients.
When needs change, you'll ensure steps are taken to provide needed services, including transitions to hospital or hospice.

Ideal candidates will bring:
- Registered Nurse or Licensed Practical Nurse certification in New Mexico (RN or LPN in NM)
- Previous experience in supervisor level/ manager level capacity in Memory Care or Home Care work settings
- Track record of success
- Stable tenure in previous jobs
- Desire to live and work in the Santa Fe, New Mexico area
