Position Summary
The Account Executive – Home Health & Hospice is responsible for driving admissions growth by managing the referral-to-admission process for home health and hospice services. Reporting to the Vice President of Home Care and Hospice, this role focuses on building and sustaining strong referral relationships, removing barriers to timely admissions, and supporting growth and profitability goals for patient volume. The Account Executive also provides follow-up support to patients and families after admission and discharge to ensure continuity of care and satisfaction.
What You’ll Do
In this role, you will be a key contributor to growth in a well-established, competitive market, serving as a trusted partner to physicians, hospitals, and community providers.
Key responsibilities include:
- Develop and execute a territory-specific growth plan to meet or exceed monthly admission goals
- Build, maintain, and strengthen relationships with physicians, hospitals, assisted living facilities, and other referral sources
- Serve as the primary point of contact for referral partners, ensuring timely communication and high satisfaction
- Monitor referral activity, trends, and opportunities to drive consistent admissions growth
- Collaborate closely with agency leadership and clinical teams through weekly Sales and Operations meetings
- Promote agency services through networking, community outreach, trade shows, and industry events
- Facilitate the referral and intake process, including coordination of physician orders and support of timely admissions
- Ensure accurate documentation and compliance with state, federal, Medicare/Medicaid, and ACHC requirements, including face-to-face documentation when applicable
- Support scheduling and follow-up activities as needed to enhance the patient and family experience
- Maintain strong, professional relationships with patients and families throughout admission and discharge

- Bachelor’s degree or equivalent related experience
- Experience in home health and/or hospice, long-term care, or managed care environments preferred
- Minimum of one year of directly related medical or healthcare sales experience, including calling on physicians, nurses, skilled nursing facilities, and other healthcare referral partners (e.g., DME, home health, hospice)
- Community relations experience preferred
